Accrued Paid Time Off
For every 500 hours you work, you will earn 4 hours of paid time off. This can only be used for planned time off — not for call-offs or cancellations and must be used within the calendar year.
You must submit your PTO request a minimum of 2 weeks in advance. Submit requests by completing the Time Off Request Form and sending it to your scheduling supervisor via email/text/fax. You will receive an email notification that your request was received and approved.
